To our Dear Families,

It is with a very heavy heart that I am writing to inform you that A Bright Future Pediatrics at 5400 W. Plano Pkwy, Ste 210, Plano, TX 75093 will be closing our doors. Our last day of practice will be October 31, 2025.

After 24 years of serving the children and families of A Bright Future Pediatrics in Plano, with the changes in the insurance system over the past several years, it has become very clear that providing quality primary care to our children is unsustainable as an insurance based practice. More so than ever, the current state of insurance maleficence toward both patients and physicians is just mindboggling. It has been a pleasure and a privilege to watch your children grow and care for their health. We will greatly miss our relationships and are grateful for the trust you have places in us over the years. We are honored to have been your children’s Pediatricians.

To ensure your child’s uninterrupted medical care, we encourage you to begin the process of finding a new pediatrician as soon as possible. However, we would love to continue your kiddos care through October and say good-bye.

• For assistance finding a new physician: We recommend contacting your insurance carrier’s provider directory. For those of you who are self-pay, Caitlan can assist you with finding a practice with reasonable self-pay rates. You can send her an email at cmateus@abfpediatrics.com

Your child’s medical records are confidential and can be transferred to a new physician only with your written authorization. For your convenience, we will download your records onto the patient portal for you to download and print yourself. We will be working on getting these downloads completed in the next 30 days. If you log in and your medical records have not been compiled for download by October 15th and you need them now, please email us at cmateus@abfpediatrics.com and we will get this done ASAP.

If you wish for them to be sent to another physician’s office, we have a Medical Records Release Authorization Form posted on our website www.abfpediatrics.com . We will only email or fax records to your new physician. If you desire a personal copy, your children’s records will be available on the patient portal to download over the next 30 days.

To transfer your child’s medical records, please follow these steps:

1. Complete the authorization form
2. Indicate the name, address, and fax number of the new physician
3. Sign and return the form to our office by October 29. 2025

Information about medical records after the practice closes:

• Custodian of records: After October 31, 2025 patient medical records will continue to be stored securely with Marcy L Berry, MD
• Contact for future requests: Call 972-208-8668 or Fax 972-208-3186
• Record retention: Your medical records will be retained in accordance with state law.

We will do everything we can to ensure a smooth transition of your child’s care.

A Bright Future Pediatrics is now using doxy.me to conduct telvisits. This service is a very user-friendly way to connect with your provider and requires no separate app download or login information.

Please be sure to use Google Chrome on a computer, laptop or Android device and Safari on Apple's iOS or iPad.  Other web browsers are NOT supported.

There will be a charge for all telehealth visits.  If you are unsure of how your insurance covers telehealth please reach out to them directly.  If you are doing a telehealth visit after hours and have not yet completed the telehealth authorization form please click the link below to complete and email back to the practice at office1@abfpediatrics.com.  If the physician does not have this form at the time of your visit you will be asked to give verbal consent agreeing to these terms.
